This article details a heart failure care model at North East Veterans Affairs (VA) Medical Center. The North East VA health system has been involved in quality improvement of heart failure care for many years. This involves continuous quality improvement in the full spectrum of treatment from admission through discharge and outpatient follow-up. Improving patient care is always the main goal. Assisting patients to better understand self-care concepts is key to avoiding heart failure exacerbations. Educating patients to identify problematic symptoms early and access the system for help can often avoid costly readmissions. The case study provided in this article highlights the journey of a heart failure patient treated at this VA hospital and the care coordination process, which is necessary for good patient care through use of multidisciplinary team members.

Hospitals and healthcare providers are looking for methods to reduce hospitalization rates and improve patient outcomes for patients with heart failure (HF). Using behavioral approaches to increase patients' confidence in their abilities to perform self-care is 1 such approach. Motivational interviewing is an empirically validated modality that has shown promise in improving motivation to change and confidence in the ability to do so.
This case represents a number of themes common in the management of care for a HF patient and explores a 4-session brief motivational interviewing approach to address these themes. The manner in which patient frustration is linked to hospitalization is discussed along with possible ways to address problems in self-care behaviors.
With the use of this brief motivational interviewing approach, the patient reported an increase in her motivation and ability to change and developed a postdischarge plan for incorporating self-care behaviors in her daily routine.
Motivational interviewing may be an effective method of increasing the self-care behaviors of patients with HF.
The purpose of the online Nursing Journal Club is to facilitate the use of evidence-based practice in providing excellent nursing care.

It should take you about two hours to read the articles, take the quiz, complete the evaluation and participate in the discussion.
- As part of the requirements to earn CE credit, you must participate in a discussion about the articles. This discussion will take place via Google Docs/Drive. A Nursing Journal Club designee will generate the discussion. You must participate in the discussion at least 1 time by November 30 to earn CE credit. The discussion is intended to clarify or provide fellow participants with further information on this topic. Suggested discussion focus:
• Your experience caring for patients with heart failure
• How you plan to use the information provided in this article in your practice
• Respond to another participant’s comments
