Literacy, readability and cultural barriers: critical factors to consider when educating older African Americans about anticoagulation therapy.

The number of patients on anticoagulation therapy has increased dramatically over the past two decades. Yet, few studies have examined the psychosocial barriers of low literacy, culture and inappropriate patient education materials used to teach older African Americans about their anticoagulation therapy. The aims of this study were to investigate literacy levels among older patients, and evaluate the readability and determine the cultural sensitivity of written information used in an anticoagulation management clinic. A descriptive, correlational design was used. Patients' (n = 62) knowledge levels and the readability and cultural sensitivity of written materials were examined. The Rapid Estimate of Adult Literacy in Medicine (REALM) was used to measure reading skills of patients, while the SMOG formula (a formula for assessing readability) was used to test the readability of written educational materials used in an Anticoagulation Management Clinic. A Knowledge Information Profile, developed for this study by one of the authors (Wilson), was used to measure patient knowledge about warfarin, medication side-effects and food sources of vitamin K. A modified, culturally sensitive and easy-to-read pamphlet was used as an alternative teaching tool in the study. The results of the study revealed the average self-reported for highest grade completed in school was twelfth grade; however, the actual mean reading skills were between seventh and eighth grade. The readability of the written information was three to four grades higher than patients' reading abilities. None of the patient education materials were culturally sensitive. This study underscores the importance of having information that is understandable and culturally relevant to prevent the outcome of internal bleeding. Nurses have a vital role in educating patients and ensuring that teaching materials are appropriate for the target population.

As a health-care–associated condition, VTE is receiving increased attention from patient safety experts, the Agency for Healthcare Research and Quality (AHRQ), and the Centers for Medicare & Medicaid Services (CMS).
The purpose of the online Nursing Journal Club is to facilitate the use of evidence-based practice in providing excellent nursing care.

- Identify at least two common consequences of venous thromboembolism

There are 3 parts to earning CE credit for these articles. All components must be completed between December 1 and January 31, 2015:
- After reading the articles, there is a short post-test on the Learning Link. To find the Learning Link module, open and login to Learning Link, click View course catalog,
enter PRCH4028 in the Search For box, leave the default category as All courses and then click the Search button to find the Dec/Jan 2014 journal club module. You must complete the post-test on or before January 31 and score at least 80% correct to earn CE credit.
- An evaluation of this learning opportunity is required to obtain CE credit. A link to the Survey Monkey evaluation will be found at the end of the post-test in Learning Link. It should take you about two hours to read the articles, take the quiz, complete the evaluation and participate in the discussion.
- As part of the requirements to earn CE credit, you must participate in a discussion about the articles. This discussion will take place via Google Docs/Drive. A Nursing Journal Club designee will generate the discussion. You must participate in the discussion at least 1 time by January 31 to earn CE credit. The discussion is intended to clarify or provide fellow participants with further information on this topic. Suggested discussion focus:
• Your experience caring for patients taking anticoagulants
• How you plan to use the information provided in these articles in your practice
• Respond to another participant’s comments
To add some extra excitement to our Journal Club, we will be offering a $25 gift card in a raffle drawing every quarter for members that earn CE credit. The current raffle covers the months of October, November and December and the raffle will take place at the end of December. Be sure to participate so your name will be included in the drawing. As an additional perk, if a participant encourages another Journal Club member to join in the discussion (and the referred member states the referring member’s name in their discussion), we will add the referring member’s name an additional time in the drawing. Remember that both members must earn their CE credit for the month for the referring member to be entered an additional time in the drawing.
The winner will be notified via e-mail and the gift card will be sent to them within 2 weeks after the close of the month.
